Our Registered Nurse will conduct an assessment of your family member and thoroughly check your home and living situation to ensure a safe environment. We will work with the family and caregivers to address safety concerns, medical issues, medication management, nutritional support, behavioral concerns and propose activities to enhance the well-being of your family member.

John Wright - Business Manager

  • Founded Transitions Senior Living Inc. in 2015.
  • Attended Miami of Ohio University Business School in Oxford, Ohio.
  • Has thirty-five years of experience in private business management in British Columbia.
  • Oversees the corporate and day-to-day operations of Transitions Senior Living Inc.
  • Member of Seniors First, British Columbia Information, Advocacy, and Support for B.C. Seniors
  • Member of the Senior Services Society of British Columbia.
  • Member of the Council of Senior Citizen's Organizations of British Columbia.

Nancy Polyhronopoulos RN, BSN - Director of Care

  • Graduate of the University of British Columbia School of Nursing.
  • Specialized education and experience in neurosciences and gerontological nursing.
  • Throughout her career, she has worked as a staff nurse, clinical educator, and manager in both acute and long-term care facilities.
  • Clinical instructor in the BSN program at UBC and taught clinical labs and nursing skills.
  • Extensive case management experience in community settings.
  • Expertise with Movement Disorders and DBS (Deep Brain Stimulation).
  • Presenter at numerous neurosciences conferences both locally and internationally.
  • Member of the following associations:

Your Comprehensive Physical and Psychological Assessment and Individualized Plan of Care will include:

  • Monitoring Health Status.

Registered Nurse (RN) accompanied Doctor and Medical Appointments

Our goal is to take the anxiety, uncertainty and unknowns out of you or your family member's medical and healthcare appointments.

Our registered Nurse (RN) will meet you or your family member at your residence, transport you to your appointment, sit in on the appointment, ask questions and provide a written detailed summary of the visit.

Following the appointment, the RN will ensure lab work is scheduled, prescription changes are filled, and any other pertinent tests or suggestions made at the appointment have been addressed.

Prior to the Medical Visit, the RN will:

  • Liaise with the client or family member to obtain medical history, medications taken and any concerns and questions they would like addressed at the appointment.

During the medical visit, the RN will:

  • Liaise with the physician/healthcare provider to ensure all medications, medical history and records are up to date.
  • Observe the appointment and take notes.
  • Pose questions to the physician (provided by family or client, as well as those the RN feels are necessary to clarify recommendations and treatment).
  • Observe the client and poses additional questions to ensure the client understands the information being presented.
  • Ensures the client leaves the appointment with an understanding of what was discussed by the physician, as well as any treatment or recommendations that were made.

Post Appointment the RN will:

  • Ensure lab work, tests and procedures are scheduled.
  • Prescription changes are dropped off at the clients pharmacy, and delivered in a timely fashion.
  • Provide a detailed written summary of the visit.
  • Meet with client or family to discuss the appointment and any treatment recommendations.

Transportation Services:

  • Transportation Service is included with all RN Accompanied Medical Appointment Liaison services.

Respite Care

  • We Offer 24-Hour In-Home Respite Care for Those Who Need Assistance When a Caregiver Needs a Rest.
  • Those Who Can Benefit From Respite Care May Include Post-operative Patients Returning Home From the Hospital, Those Caring for a Family Member With Dementia, or Caregivers Going on Holidays or Business Trips.
  • We Also Provide Overnight Monitoring and Care as Requested During Post-operative Recovery.

Surgical and Post-Operative Care

  • Accompaniment to All Pre-operative and Specialist Appointments to Ensure All Questions and Care Needs Are Addressed.
  • Transportation to and From Appointments and Hospital or Surgical Centers.
  • Liaise With Treatment Teams for Discharge Planning.
  • Ensure Post-operative Supplies Are Set Up.
  • Liaise With Physiotherapists, Occupational Therapists, or Other Healthcare Supports as Needed for Recovery.
  • Collect Prescriptions and Ensure All Post-operative Medications and Supplies Are Set Up.

Transportation Services

  • Making sure you never have to worry about getting to and from your important medical appointments safely and on time.
  • Medical and dental visits.
  • Hospital visits.
  • Pre- and post-operative travel requirements.
  • Pharmacy pick-ups and drop-offs.
  • We will take you safely home after being discharged from the hospital and to any required follow-up appointments.
  • Our transportation services driver will remain at the location of your medical appointments for the duration, ensuring you will always be driven safely back to your residence.
  • Our registered nurse will accompany you to all relevant medical appointments.
